SSDI Disability Application

In order to receive SSI or SSDI you will need to fill out an application. The application for SSDI can be filed online or in person at your local Social Security office. To view and start on your application, visit the www.ssa.gov website. The application for SSI must be filed in person as there is no online alternative available yet.

SSDI Work History Report

In addition to the Disability Application, you will also need to fill out a Disability and Work History Report which will ask for information about your medical condition that has stopped you from working. You will need to access your medical records to obtain some of the information such your doctor’s name and addresses, hospital and clinics where you have been treated and medical tests that you have undergone. You will also need to include a detailed account of the work you have done in the past 15 years.

When filing for your SSDI online, you can do both the application and the work history form together in succession. When filing for SSI you can choose which section to fill out first. You can complete the work history section online or in your local Social Security office.

SSDI Disability Benefits Appeals Process

If your application is denied, then you have the right to appeal the decision and try again. There are two different steps involved in any appeals case:

Request for reconsideration – you will need to first fill in a request for reconsideration within 65 days of the application denial letter. There are two different ways to do this – either online or in person at your local Social Security office.

Request for hearing – you will also need to fill out a request for hearing which can also be done either online or in person at your local Social Security office. Again, you have 65 days to complete this form in order for your appeal to be accepted and your applications to be re-processed.
